Help Me Grow - Early Intervention

Overview


The Help Me Grow (HMG) Early Intervention provides family-centered services for infants and toddlers ages birth to 3 with developmental delay, disability or a medical condition likely to result in a delay or disability. If eligible, the Early Intervention Specialist will coach the family on strategies to enhance their child's development and provide services in the home.

Do you have a concern about your child's development?

• HMG Early Intervention (EI) service needs are determined by a multidisciplinary team who evaluates and/or assesses the child's developmental needs.
• The EI Team is led by a service coordinator who will work to assist and enable a child with a delay or disability and the child's family to receive services and rights as afforded under the Individuals with Disabilities Education Act (IDEA).
• Children and their families will transition when they complete needed services or when the child turns age three, with supports to move on to the next phase of growth and development.

 

What information will I need to make a referral?

• Child's name and date of birth
• Parent or legal guardian's first and last name, address, email or phone number
• Child's county of residence

 

What happens next?

The parent will be contacted to ask about their interest in the voluntary program. Nothing will occur without the family providing consent for participation. With parent consent, HMG will provide any professional referral source with a follow up communication.

Eligibility Requirements


• Child is under age 3
• Child has a delay on the standardized evaluation, has a medical diagnosis on the HMG list, or has a medical diagnosis likely to result in a delay or disability
• Child and family show a need for service
• Family agrees to participate in the HMG program
• No income guidelines required
